In-network Insurance Benefits

If Tatiana is an in-network provider with your insurance company, a claim for your lactation consultation will be submitted on your behalf to your insurance company.

Insurance companies currently accepted:

  • Blue Cross Blue Shield of Arizona
  • Arizona Care Network

The Affordable Care Act (ACA) mandates that lactation support be a covered benefit without any cost-sharing by the mother. Some plans, however, do not meet the criteria of the ACA, and in these situations it is possible that your insurance company will apply the billed charges to your deductible, or assign you a co-pay or co-insurance responsibility.

It is highly recommended that you contact your insurance company before your appointment to review your benefits for "lactation coverage as preventative health care under the Affordable Care Act."

You are financially responsible for any co-insurance, deductible, co-pays, or other fees that insurance does not pay and will receive an invoice for payment, if any is due, after your benefits have been processed.

Out-of-network/Self-Pay Clients

Out-of-network claims for some insurances may be submitted on your behalf, if requested. Reimbursement cannot be guaranteed. 

A deposit of $175 will be collected at the time that services are provided. Cash, check, credit/debit cards, and HSA/FSA cards are accepted. 

If your insurance company provides payment, you will be reimbursed the amount paid up to your deposit amount. If insurance denies the claim, your deposit will serve as your payment in full.
