In-network Insurance Benefits

If I am an in-network provider with your insurance company, a claim for your lactation consultation will be submitted on your behalf to your insurance company.

Insurance companies currently accepted:

  • Blue Cross Blue Shield of Arizona
  • Arizona Care Network Intel Connected Care Plan

The Affordable Care Act (ACA) mandates that lactation support be a covered benefit without any cost-sharing by the mother. Some plans, however, do not meet the criteria of the ACA, and in these situations it is possible that your insurance company will apply the billed charges to your deductible, or assign you a co-pay or co-insurance responsibility.

It is highly recommended that you contact your insurance company before your appointment to review your benefits for "lactation coverage as preventative health care under the Affordable Care Act."

You are financially responsible for any co-insurance, deductible, co-pays, or other fees that insurance does not pay and will receive an invoice for payment, if any is due, after your benefits have been processed.

Out-of-network/Self-Pay Clients

If insurance benefits are not being used, appointment fees will be collected at the time that services are provided. Cash, check, credit/debit cards, and HSA/FSA cards are accepted. 

Same-day fees are as follows:

  • 90 minute office visit-$175

After your visit, you will be provided with an insurance superbill (receipt) that can be submitted to your insurance company to seek reimbursement. Reimbursement cannot be guaranteed.
